After clicking on the appropriate file, if you are unable to open the document you can download a free app called Adobe Fill and Sign from the Google Play Store or iOS Apple App Store to complete, sign, and return. Everyone must fill out either the new female or new male patient paperwork and if you have had any labs drawn within the last year please fill out the medical records release as well so we may obtain those prior to your appt.  Once completed you may submit the documents via email (inshapewv@gmail.com) or fax (681-238-5700) to expedite the scheduling process and your wait in the office on the day of your appt.

 

Upon receiving your paperwork we will reach out to you within 24 hours with an appointment date and time. If you have not heard from us within that time frame please reach out to us (office number 681-238-5258 or office cell 681-238-8700) to ensure we received the paperwork.
